The word ectasia defines an abnormal dilation of a hollow organ, glandular canal or vessel. In the context of the urinary tract, pielocalicial ectasia or hydronephrosis is understood, the occurrence of a dilation of the renal collecting system, that is, the calyxes and renal pelvis. This condition, with a common diagnosis in pediatric patients, may be uni or bilateral and is due to multiple factors, which may be obstructions or emigrations, responsible for urine accumulation and consequent increase in renal volume. This may lead to severe insufficiency of this organ if the underlying pathology is not treated in a timely manner. In this condition, it is noted the importance of early diagnosis and treatment so that the prognosis can be the best possible in the face of each case and for the kidney and the functioning of the entire urinary tract to be preserved. In this sense, this study aims to describe a clinical case of hydronephrosis with unsatisfactory evolution due to late diagnosis. At the time (of diagnosis), the advanced state of the pathology made nephrectomy necessary – an extreme conduct for these situations. Report: A 40-year-old female patient diagnosed with dilation of the pielocalicial system on the right, at 18 years of age, who evolved unsatisfactory and culminated in right nephrectomy at the age of 21 years, due to loss of renal function resulting from late diagnosis. Conclusion: According to the literature, the treatment recommends early intervention in the underlying disease causing dilation to prevent worsening of the condition and avoid a worse prognosis for the patient.
