Challenge in the treatment of impacted permanent molar: case report

Impaction refers to the failure of a tooth to appear in the dental arch due to lack of space or the presence of a barrier in its eruption way. The impaction of a permanent first molar is rare and usually occurs in the jaw arch. The aim of this study was to report a clinical case of unilateral traction of the first permanent molar in the jaw region. The present work presents a clinical case of a female patient, 11 years old, diagnosed with the impacted permanent first lower molar right. The orthodontic traction of the impacted tooth was performed with surgical access, installing bonding orthodontic buttons on the vestibular and lingual face of the molar, traction using string wire and elastic chain adapted to the hooks of the modified lingual arch appliance, thus allowing anchorage for traction of that tooth. The use of orthodontic appliances for the traction of impacted teeth present lots of reports of success in the literature, as long as they are diagnosed early and with roots favorable to traction. If unsuccessful, the recommendation is for prosthetic replacement or occlusal restoration when possible.
