The article describes the modeling of a hybrid care platform based on biotelemetry and mHealth capable of detecting points of intervention or levels of application of preventive measures in the health-disease process. We sought to understand pathologies that do not have direct measurement equipment, such as glucometers for diabetics. A survey of requirements was carried out for the application of analytical algorithms in the predictive category, so that the system can contribute to the monitoring of users with depressive symptoms. A model was built to capture the data and define the measurement parameters to support self-care and increase the user's sense of security. It is possible to improve user conditions by analyzing their routines, monitoring symptoms in the initial phase, in addition to promoting the reduction of high costs with hospital care, hospitalization, readmissions and readmissions.
